PUPPY HEALTH AND TRAINING INFORMATION

VACCINATIONS
The first full year of vaccinations, including Rabies, is included in the adoption fee. If not already administered, it is your responsibility to bring your puppy to ANIMART’s vaccination clinics to complete necessary vaccinations at no additional charge. See the Pet Health Record for your pet’s vaccination schedule. Vaccinations include: Canine distemper, adenovirus type 2, parainfluenza, coronavirus, parvovirus and bordetella bronchiseptica. In addition, your puppy was tested negative for Parvo virus. (Not included: Lyme and Lepto)
PARASITES
All puppies are routinely dewormed and tested for internal parasites, including giardia and coccidia. See the Pet Health Record for history. It is your responsibility to administer any needed treatments and return to ANIMART for any additional lab work as indicated on the Pet Health Record. There are no additional veterinary or treatment costs for existing conditions through ANIMART’s Veterinary Wellness Center.
While at ANIMART, puppies are started on monthly heartworm, flea and tick prevention, as indicated on the Pet Health Record. ANIMART recommends keeping your new pet on a year-round preventive program and offers many low-cost options to you.
SURGERY
Your new pet has been spayed or neutered. It is your responsibility to bring your puppy to ANIMART for suture removal – at no additional cost – if indicated on the Pet Health Record.
OTHER HEALTH CONDITIONS
Your new puppy has received a Certificate of Health by ANIMART’s veterinarian. Any conditions noticed on the health exam and treatments are noted on the Pet Health Record. Current medications will be dispensed to you at the time of adoption. It is your responsibility to administer any needed treatments and return to ANIMART for any additional exams as indicated on the Pet Health Record. There are no additional veterinary or treatment costs for existing conditions through ANIMART’s Veterinary Wellness Center.
TRAINING
Training and socialization is one of the most important things you can do for a new puppy when it joins your family. Sound training and great social skills are as important as vaccinations and parasite prevention for the health and wellbeing of your new puppy. Puppy training classes are an excellent way to play and bond with your puppy, while at the same time, learning strategies for communicating and teaching your dog desirable behavior patterns.
LIMITED HEALTH GUARANTEE
ANIMART offers a Limited Health Guarantee to ensure that you and your new puppy get off to a great start. Your new puppy is guaranteed to be free of any known infection or disease, unless otherwise noted at time of sale on the Pet Health Record, for a period of 14 days. Congenital and hereditary defects are guaranteed until one year of age (except for Furry Purry Rescue puppies).
Any existing condition will continue to be treated and monitored by ANIMART’s Veterinary Wellness Center at no additional charge until declared healthy by the attending veterinarian. It will be your responsibility to administer treatments and return for visits as directed.
If your puppy is diagnosed with a congenital or hereditary defect before one year of age, you must notify ANIMART within 48 hours of this diagnosis and provide a written veterinary diagnosis. ANIMART’s Veterinary department will verify this diagnosis with the attending veterinarian and may require you to bring in the puppy for an examination. Once confirmed, if a non-curable defect, ANIMART will refund you the purchase price of the puppy; if a curable defect, ANIMART’s Veterinary department will, at our option, either perform the needed treatment at no cost to you or refund you up to the purchase price of the puppy towards your veterinary bills. (Diagnosis and copies of bills required.)
ITEMS NOT COVERED
RETURN POLICY
ANIMART knows that even the best planning is met with unforeseen circumstances, hindering one's ability to fulfill a lifelong promise to an ANIMART pet. For cases such as these, ANIMART offers a 48-Hour Return Policy: within 48 hours of the purchase of an ANIMART puppy, ANIMART will gladly refund a customer's money less a $150 administration fee.
Bringing a puppy back to ANIMART is emotionally hard on the pet owner and disorienting to the pet. The policy's administration fee is in place to discourage impulse sales and "rent-a-pet" purchases of ANIMART pets. The fee also pays for associated veterinary care and re-entry of the pet into an ANIMART store.
Besides matching the right pet and people, our goal is that no ANIMART pet ends up homeless or burdens an animal shelter. If an ANIMART pet owner finds he or she can no longer keep their commitment to an ANIMART pet, we can help find a new home through ANIMART’s Furry Purry Rescue program or Pet Connection.
REFUNDS
Once approved by ANIMART’s Veterinary department and or adoption counselor, refunds will be processed within 14 business days by credit card or check.
Adoption Agreement
_____ I understand my pet’s medical, vaccination and treatment needs and agree to continue any needed treatments, vaccinations or tests as indicated on the Pet Health Record.
_____ I have read and understand ANIMART’s Limited Health Guarantee, including my responsibilities, items not covered, return and refund policies.
_____ I completely understand that Animart is not responsible for any allergies or zoonotic diseases that members of the household may have or develop with the adoption of my new pet.
_____ I also understand that Animart cannot guarantee against any type of behavioral problem. I realize that it is my responsibility to investigate these types of problems and find my own solution.
_____ I realize that many pets given as gifts are not truly appreciated and may not be the love of a lifetime. If this puppy is a gift, that person is either: 1) my child and I accept all responsibilities of ownership 2) an adult signing this agreement as the owner.
_____ I am 21 years old (verified by ID), I either own my own home or have the permission of my landlord to have this pet in my apartment (verified by copy of lease or written permission by landlord), and all persons living with me have agreed with my adoption and responsibilities of owning this pet.
_____ When adopting a pet, I understand that it is like adding a new member to the household to be a permanent part of the home and family. I am committed to providing my new pet a healthy, loving long-term home.
